Ultrasound Therapy at PYONG Rehabilitation

Table of Content

Therapeutic ultrasound involves transmitting high-frequency sound waves into tissues to produce “deep thermal effects” and/or “mechanical effects.” These mechanisms help relax muscle tension, reduce irritation in tendons and fascia, and support blood circulation in affected areas. It is commonly used as a supplementary tool alongside therapeutic exercise, stretching, or disease-specific rehabilitation programs. It is ideal for seniors and working professionals suffering from joint, tendon, or muscle pain—such as knee osteoarthritis, rotator cuff tendinopathy, and neck/shoulder pain—as well as those recovering from surgery or accidents.

At PYONG Rehabilitation Penthouse Clinic (Gaysorn Tower, 11th Floor, near BTS Chidlom), every patient undergoes a thorough history taking and physical examination. A personalized treatment plan is designed by a Physiatrist (Rehabilitation Physician) in collaboration with a physical therapy and multidisciplinary team for maximum safety and efficacy. We also integrate advanced recovery technologies such as PMS, High-Power Laser, Focused Shockwave, and Dry Needling as appropriate for each individual.

What is Therapeutic Ultrasound?

Therapeutic Ultrasound uses a device head to transmit high-frequency sound waves (typically 1–3 MHz) through a cooling conductive gel into the tissues beneath the skin. These sound waves safely vibrate cells and fluids, resulting in two primary effects:

  1. Thermal Effect: Increases tissue elasticity and reduces spasms or stiffness.

  2. Non-thermal (Mechanical) Effect: Includes processes like acoustic streaming and cavitation, which assist in soft tissue repair and microscopic wound healing.

Physicians choose between Continuous mode (focusing on heat) or Pulsed mode (focusing on mechanical effects) based on the treatment goals and the depth of the lesion.

Why Choose PYONG Rehabilitation?

  • Advanced Technology: We utilize state-of-the-art ultrasound devices from the USA featuring “Dual-Frequency” mode (1 MHz for deep and 3 MHz for superficial tissues simultaneously), allowing for flexible depth control.

  • Physiatrist-Led Care: A Rehabilitation Physician supervises every step—evaluating the root cause, selecting specific parameters, and sequencing integrated tools (PMS, Laser, Shockwave, or Dry Needling).

  • Comprehensive Multidisciplinary Programs: Integrated with physical therapy, occupational therapy, balance/gait training, and nutritional or psychological counseling as needed.

  • Convenient Location: Located at Gaysorn Tower, 11th Floor (BTS Chidlom).

  • Systematic Follow-up: Monthly goals are set and plans are adjusted based on actual response for sustainable and safe results.

How it Works

High-frequency waves cause cellular vibration, controlled tissue temperature increase (in continuous mode), and stimulate microcirculation. Upon arrival, a physician assesses the painful area and sets parameters (frequency, mode, duration).

The Procedure:

  1. Clean the skin and apply conductive gel.
  2. Move the device head in steady, broad circular motions.
  3. Adjust energy levels based on the patient’s sensation (should feel comfortably warm, never burning).
  4. Immediate follow-up and design of supplementary programs (stretching or strengthening). The duration is typically 5–10 minutes per area.

Conditions Treated

  • Knee pain from Osteoarthritis.
  • Rotator cuff tendinopathy / Frozen shoulder.
  • Office Syndrome (Neck, shoulder, and back pain).
  • Muscle tension and thickened fascia.
  • Chronic tendonitis (Achilles tendon, Tennis Elbow).
  • Post-operative recovery and scar tissue management.
  • Trigger points (often used alongside Dry Needling).

Academic Evidence

Research on Knee Osteoarthritis (Knee OA) indicates that both continuous and pulsed ultrasound can help reduce pain and improve joint function when used as part of a comprehensive rehab program involving exercise and lifestyle modification. (Sources: Oarsijournal, PubMed, PMC).

Frequency of Treatment

Typically 1–3 times per week for 2–6 weeks. It serves as an “adjunct tool” to facilitate better stretching and movement patterns. As the condition improves, the frequency may decrease in favor of active exercise programs.

Expected Results

  • Short-term (First week): Many patients feel immediate relief from stiffness, a soothing warmth, and slight improvement in range of motion.
  • Long-term (4–6 weeks+): Significant pain reduction and improved outcomes in chronic conditions like Knee OA or Tendinopathy when combined with exercise.

Contraindications & Precautions

For safety, avoid ultrasound on the abdomen/lower back of pregnant women, areas with cancer, acute infection, or vascular thrombosis. Use with extreme caution near the eyes, heart, and growth plates in children. For those with pacemakers, avoid the chest area. At PYONG, we perform a strict sensory test and medical screening before every session.

FAQ

Q1: How is it different from diagnostic ultrasound?

A: Diagnostic ultrasound is for imaging; therapeutic ultrasound uses different parameters (1–3 MHz) to treat and heal tissue.

Q2: What does it feel like?

A: It usually feels comfortably warm. If it feels hot or stinging, notify the therapist immediately.

Q3: Can it be used with topical medication?

A: Yes, this is called Phonophoresis, which helps deliver medication through the skin.

Q4: Can I receive treatment if I have metal implants or prosthetics?

A: A physician must evaluate this carefully, as certain materials or bone cements may heat up more than normal.
